Anyone else having flaky SD card read/writes (requiring reboot to fix)? - LG Nitro HD

Lately I'm noticing that my Nitro keeps losing read/write access to the SD card (says SD card is unmounted and ready to be removed. If you click to mount it says the card is damaged) and I have to reboot for it to work again.
I'm wondering if I didn't hose my SD card during attempts to root. I went through dozens of attempts to root with the SD Card mounted (supposed to be unmounted) and would force close the SuperOneClick app when it locked up on mounting the SD.
I'll give it a good formatting I think, hopefully that clears things up.

[Q 4.21.11] Blank SD Card

So it's possible I might just be dumb and screwed up, but I think I might have reformatted something that I didn't mean to reformat.
I wanted to wipe my SD card because I found that there were random pics appearing in my gallery of people I've never seen before in my life (got the phone second hand but popped the SD card in that I used with my old Fascinate). Opening up My Computer, there were two different removable discs that were the SD card inside my phone and what I now presume to be internal storage. I backed up everything I wanted and wiped both the SD card and the other storage device via my laptop.
Here is the problem. Whenever I boot the phone up, there is a notification that says my SD card is either blank or not in a compatible format... It asks if I want to format it, I click OK, and everything seems fine... Until I have to reboot the phone and it does the same thing all over again. This is getting to be an annoyance because I don't want to keep deleting everything on my SD card whenever I reboot my phone.
Can anybody help me fix this?
Okay I definitely did something to the internal storage. I tried taking my SD card out and rebooting it and I am still getting the blank SD card notification. It won't let me format it. What do I do?
Try connecting the sdcard to your computer through a usb adapter, or any alternative to using the phone's USB-mount. Format it with FAT32. Put it back in your phone and see if it works. If not, you might have a corrupt sdcard. Try a different one if available.
What directory were the pictures appearing in? It's possible they were cached from the browser or some social app.

phone dont recognize sd after reboot

My Droid Inc doesn't recognize my SD card after reboot or shutdown and erasing photos. Anyone have a fix for this besides getting new phone?

I have and it does the same. I can unmount card and then mount it and it reads fine
I had to reformat my sd card within the phone when I had this problem. It seemed to have fixed the issue.

[Q] SD card refuses to recognize!

On my DInc, my SD card refuses to be read even though there is a card in the slot. If I boot to recovery, I can go under Mounts/Storage and Mount USB Storage to copy my ROMs etc to it from Windows Explorer.
But as soon as I boot into the Dinc, the SD card says "Preparing SD card" followed immediately by "SD Card Removed". What's the deal with this?
I've used a great number of ROMs and it simply hasn't worked in a while. I'm currently running Clutch 1.0.
Any ideas how to get my SD card working from the phone again? To reiterate, I can access the SD card if I reboot to recovery and access it under Mounts/Storage. Just not when the phone is booted up for use.
I think your best bet would be to reformat the sd card through a pc to FAT32. Make sure to copy all the sd card contents to a computer before formatting it. See if this helps.

[Q] SD Cards

I tried to format my sd cards tonight via computer format option. However my computer shut down in the middle of the process and now I can't see my sd cards anymore. I can't mount it at all. When I go into the recovery to wipe it, twrp says failed.
Does this mean it's completely dead?

and for my external, i used a card reader, plugged to pc, and reformat it.
also checked both cards for errors.
whew. i am never gonna reformat my cards again. ever.

Encrypted SD Card Corrupted from phone

Hi,
I have a 128GB Sandisk SD Card. and had it setup as internal storage.
This morning. I noticed that some of my apps installed in my SD card are missing. so I checked my SD card from my file explorer. turns out it doesn't see the SD card. So I restarted my phone and my SD card still did not get recognized.
Then I removed the SD card and inserted it after restarting. then a message of
Code:
'This device doesn't recognize this Sandisk SD card.'.
When formatting it. I got the

'command '20 volume partition disk:179,64 public failed with 400 XX Command failed'
Tried putting the SD via OTG and formatting it via that way and I am still getting an error for it.
I did a few research about the error, and said that I can try and format it via PC. then I did go to my PC to format it. On my computer it shows 2 partitions
Tried doing the normal formatting for both of it. One shows 16mb and the other as 119GB. but both doesn't continue and I am having this message
Tried looking at it via Disk Management and can only perform this
And getting this error when trying to do so
Also tried doing it via diskpart and still getting this.
I need help on this. I feel that there is something I(We) can do to salvage this SD card. Any help would be appreciated.
Also, I haven't tinkered on my phone so it's not rooted/flashed yet (hoping that i wont be needing to do this). And the SD Card is authentic as I bought it from a reliable store
