Make a backup of your current ROM to create the TWRP folder structure
Copy the unzipped file to your twrp folder
Enter to twrp recovery and wipe data (factory reset)
Now restore the stock backup
After the restoring proccess click DO NOT INSTALL when ask you to root the ROM.

Instructions to apply the OTA:
→ Restore latest nandroid backup posted in OP
→ Flash stock recovery (via fastboot)
→ Copy the OTA file in the download folder of your phone
→ Reboot to recovery ( I use adb reboot bootloader and then with the volume buttons select recovery)
→ Wait for the warning triangle, keep pressing the volume button and touch and release the power button, the options should appear.
→ Select apply from phone storage and then go to the download folder where you copied the OTA file, select it and hit the power button to apply the OTA.
→ Let it run and follow the instructions at the end to reboot the phone.

You have to relock the bootloader :
"fastboot oem lock"
Then "fastboot reboot"
and run the RUU as administrator.

One question how do you flash radio.img , I been away from HTC device for awhile and does it wipe my phone?

Flashing a radio won't wipe your device.
Copy the img to your adb folder, enter your phone in fastboot mode, connect your phone to pc with usb cable, open a command window in your adb folder and type: fastboot flash radio radio.img*
or in this case: radio_firmware.img
and fastboot reboot.

Couldn't flash that radio for some reason.
I'm supercid tried it that way. Even tried TMO cid and I got an error.

Just tried to flash the stock recovery and got an error in twrp. S-off as well.

Just rooted/s-off my device and decided to factory reset it to start using it for regular usage. I accidentally clicked erase internal storage in tmrp recovery and deleted the whole os on the phone as well. Is there any way to side load a rom with recovery to get the phone to boot up again or is my phone practically a brick till a RUU is released. Anyway I can sideload you backup? This has never happened to me some I'm nervous as hell

Has anyone have success pushing the radio.img ,I'm at work now so I can't test it.
Update: I get a message" FAILED (remote: not allowed)"

Day 2 with my T-Mobile M8 and I unlocked the bootloader, flashed TWRP, and attempted to S-off with no luck as Firewater doesn't work with all HTC One M8's at the moment. So my bootloader says **Tampered** and **Unlocked** ... Still S-On
It wasn't until after that I noticed I have a defective display and have to take the device back. Read the OP and I'm not 100% clear what I need to do to get my unit back to 100% stock with the bootloader back to normal. Any assistance would be highly appreciated.

In the OP you'll find the stock TWRP nandroid backup and the stock recovery.
Download the nandroid backup,extract it and copy it to the twrp folder in the sdcard then restore it via TWRP recovery.
Download the stock recovery and flash it via fastboot, the command is: fastboot flash recovery "name of recovery".img
Then lock the bootloader via fastboot too. The command is: fastboot oem lock
That's it, should be good to go.

I'm running froyo and and decided to update on GB(via ota) then after updating successfully it only boots on lockscreen and after few sec it'll restart on its own, then same thing happened after i pulled out battery, it always restarts.

That's a radio failure which is very hard to fix.
Keep flashing the 2.3 RUU until it works. Search for it and there were many similar issues earlier.

it supposed to show my device but it seem doesn't see my device.

the reason is I have already tried the HTC's latest GB2.3 RUU.exe, but still gives me this:

Your phone's not detected because you need a running ROM for ADB, thus making s-off through revolutionary impossible.
Download the 2.3 RUU in my sig. It will come named as pb99img.zip. Take the zip and put it on the root of your sd-card, then boot in hboot, and select yes when prompted. It has a low chance of succeeding, but keep trying until it does.
Getting past a radio brick is very difficult. You need patience, perseverance and a lot of luck.

okay I started to download your PB99IMG.zip, one question, is there any way to install that pb99img.zip beside entering hboot? because I cant enter my hboot anymore, when I (vol- + back + pow) or (vol- +menu + pow) or (vol+ +menu+pow) or just simply turning it on, I always ended on HTC blk bckground with 4 triangles on corners(img1). but I can still access hboot via fastboot (img2).
I'll do what you said, will give you the result as fast as possible.

1. ruu_htc desire android 2.3 upgrade (gingerbread).exe - got this one attached from some thread I searched.
2. RUU_Bravo_hTC_Asia_WWE_1.19.707.7_Radio_32.36.00.28U_4.06.00.02_2_release_126179_signed.exe - from this thread http://forum.xda-developers.com/showthread.php?t=695667
3. RUU_Bravo_HTC_WWE_1.15.405.4_Radio_32.30.00.28U_4. 05.00.11_release_122704.exe - same where no.2 came from.

Extract Boot image from OTA file and flash via fastboot
That's why i asked you : is there also a problem ? So then i hoped you would answer with yes, i can't use signal, mobile data and such.....
Return to your full stock rom and indeed flash the boot.img. that OTA can't be flashed as a firmware. It's done via the recovery but if it involves a hboot upgrade it's also not possible in that way.
What you can do is flash the firmware.zip that is located inside the OTA. The OTA itself is not a firmware only !
And when you install the stock rom and flashed the firmware and the imei stays away you can't fix this at home, that needs some jtag solution that is not really accessible for the "normal" user at home. You need a repair shop or HTC for that.

Return to your full stock rom and indeed flash the boot.img. that OTA can't be flashed as a firmware. It's done via the recovery but if it involves a hboot upgrade it's also not possible in that way.
What you can do is flash the firmware.zip that is located inside the OTA. The OTA itself is not a firmware only !
And when you install the stock rom and flashed the firmware and the imei stays away you can't fix this at home, that needs some jtag solution that is not really accessible for the "normal" user at home. You need a repair shop or HTC for that.

If he can't s-off the device than its true. The partitions that needs rewriting are locked and you need a-off to reach it. And writing an imei just like that is illegal.....

First copy the file to a specific directory on the phone, then unlock bootloader and flash Philz 5.15.9.recovery because work with TWRP and CWM backups then flash the boot.img from your backup and then co in custom recovery and restore nandroid
I have one problem to solve before i can do what you told me.
I tried to instal TWRP and i install it corectly, but now i cant turn on my phone, it stuck on htc screen, white screen with htc logo, and if i turn device off it turn on immedietly without i do anything.
Can you help me to fix that, i readed somewher that i need to install right kernel to solve that problem, but i cant open sd card storage to put files ther so i can flash kernels, recovery or anything else?

Go in bootloader and run fastboot erase cache and then try to go in recovery and I mention Philz 5.15.9 recovery but it is your decision

Are you read what he say or just give one idea in the black hole. He have problem with the recovery not the nandroid he not restore it and why the hell to flash the boot.img from nandroid when he not restore it. He can't go in recover!

[Q] Is there a custom rom in ruu format?

I have an htc one m7 for verizon. ulocked bootloader and s-off. For some reason recovery software will not load. I have tried every way possible to flash twrp and cwm. they just will not load. Anyway I was able to install a RUU file through fastboot. So since I can not get into twrp or cwm to try out different roms, is there custom roms is ruu format?.
I would really like to install NuSense 8.0. Anyone have it in ruu?

For some reason your recovery partition is not getting written into. please double check that your bootloader is unlocked and you are S-off. if so then try the following options.
Via apps like flashify, rom toolbox pro try to flash the recovery image.
or via fastboot,
fastboot erase recovery (optional)
fastboot flash recovery recovery.img

I have flashed several versions via flashify and fastboot. None of them load even though they flash with no errors. I also flashed an ruu of twrp that said it installed and still can not load recovery. It tries the screen says entering recovery then goes black then right back to the entering recovery screen. The phone will stay on that screen until I power it down.

Perform this command in terminal, with device in fastboot:
fastboot erase cache
You then should be able to boot to your custom recovery.

I got this phone on eBay. It came unlocked and s-off. Is super cid ok?
I looked up supercid. Yeah the person who sold it to me on eBay must have done that. Twrp was working. I did recently convert the phone from a silver case to a red one. I wonder if that has something do with it not loading. Everything on the phone works accept the recovery.

This is so frustrating. I have tried all the suggestions and my phone will not load recovery. Thank You to everyone's input, for now the only way for me to try something new is to install via fastboot and the rom update utility. This makes no sense because twrp was working fine, I had NuSense 7.8. I saw an update to 8.0. I downloaded NuSense 8.0 and went to load twrp and it would not load. So I tried all the suggestions on here still not loading. I then Installed the ruu file that keeps me rooted and s-off kitkat 4.4.3. That worked and wiped the phone. Ok now that the phone is completely wiped and brand new install, twrp should work. NO still not loading. I should have stuck with NuSense 7.8......
Here is a picture of what happens when I enter recovery from the bootloader..

That is the RUU I flashed. It works great. My one issue is recovery not loading. Everything else works, Including flashing a ruu file via fastboot. which is why I do not understand why the recovery just refuses to load. I have tried multiple suggestions all to no avail...

Read the ruu thread OP. He has an ruu flashable recovery link. Have you tried flashing that recovery in ruu mode?

[Q] HTC M8 Wifi problems on several Roms -HELP!

I was hoping someone could help me with a problem I have.
My htc M8 was rooted and insertcoin was flashed onto it however the wifi button greyed out and wouldnt turn on in order to s-off it. I tried several different roms and they all have the same problem. I am currently running skydragon v5. I have been trying to sort this phone out for a week and I am getting absolutely nowhere so any help would be appreciated. It's on ORANG001 and is s-on.

You have old firmware. You need to have 4.19.61.2 on ORANG001 in order to have wifi on these ROM.

What's your current firmware version .. 3.33.61.9 or 2.26.61.1 ? Or even lower ?

I was trying to adb push the firmware to my phone to then install it on Recovery but when im typing "adb devices" it says list of devices attached then it is blank. I have been trying to search for solutions but I cant seem to fix it. If you have any advice it would be much appreciated. Thanks

This is usually an issue on the computer's side (PC?). Try to re-install HTC drivers, try another USB port, or another cable. Also, if you have USB 3.0, this is known to cause issues with adb connectivity (apparently, you can disable this).
Also, you need to be booted into OS or in recovery for adb to work (does not work in bootloader) and you need to have USB debugging activated in Settings.
And when you say "firmware" I think you mean a nandroid backup, based on the instructions previously given to you? Be careful using the term "firmware" here, as most folks use that term to specifically mean the firmware.zip that contains hboot, radio etc. (but not ROM).

Don't you have a MicroSD ? Put the extracted nandroid on MicroSD at the right location clockworkmod\backups\ is easier than doing that ADB push.
Okay finally got the nandroid backup installed. Then when i tried to install the OTA update it took me to CWM and failed. So i then relocked and tried again to update OTA and it took me to the bootloader with a ***Relocked*** and *** Security Warning*** any ideas ?
You need to re-install the stock recovery in order to OTA. Bootloader lock has nothing to do with it (will OTA with unlocked bootloader).

I have installed the Nandroid backup mentioned above to stock i believe. Is this the correct step or is this still not stock ? it was 1.54.61.5

Stock ROM backup and stock recovery are two different things.
You need to fastboot flash recovery recovery.img to replace the CWM that you have now. Inside the backup zip, there is a recovery.img .. take this out and fastboot flash it.
You need to unlock bootloader to flash it.
when using the command "fastboot flash recovery recovery.img" using the recovery.img from the unzipped nandroid folder the process runs and it says okay with no errors then when i try go into recovery the red triangle and exclamation mark appear again

Yes that's stock recovery. Perform the ota now
Because that is stock recovery. You don't need to get into recovery.
Just reboot, check OTA, download then select install.
Let the system do the installation on its own

Don't have your model 816, mine is single SIM a5_chl but this forum link had some answers for someone else with your model. Luckily your not really "hard bricked" because you can still enter hboot/fastboot but I understand why you say that. Hard bricked is when your phone is literally an expensive paperweight. Luckily you still have access to your boatloader and that's where most problems can get fixed. By the way what specifically happened that caused this issue? You're boatloader is locked and your not rooted so pure stock right with stock recovery but you can't get into recovery?

Below is a link to stock RUU's for yours and other models but you'll need stock recovery installed and working before they can be run. Hopefully your stock recovery is still there just need a different way to enter it i.e. Boatloader or key presses. Only flash your specific versions firmware.Try the one your were currently on before trying one older.

So you went into fastboot mode and when you press your power button to select hboot what happens?
Below is a link to a forum for the a5_ul with a link to the stock recovery. Verify this is your version then try flashing it using ADB/fastboot on your P.C. and your phone in fastboot mode. Do you have USB debugging selected in your phone developer options settings?

i cant turn my phone on even i dont have htc logo start in hboot yes i have now will try to flash ;] i think i have debugging but my device show is only in fastboot and dont list my device only fastboot , BUT IM LOCKED BOOTLOADER AND S-ON CRAZY STUCKING IN BOOT MENU ;] CAN I INSTALL MANUALLY RECOVERY ? BECAUSE I ADB DONT LISTED MY DEVICE

Did you have the HTC drivers installed on your P.C. or Linux computer?
flashing it using SDK Toolkit /ADB fastboot is doing it manually. The other method is to name the file a specific way so that your phone recognizes it but I have never used this method before. I downloaded the portion of the SDK Toolkit I needed, ADB/fastboot and had the HTC Windows drivers installed from HTC sync manager then put the recovery zip file into my ADB folder in my P.C. then opened command prompt from my ADB folder and typed fastboot flash recovery"name of zip file".img with my phone in fastboot and it flashed. This is what you should try and I'll link some instructions below. Good luck.

Also if ADB isn't recognizing your phone and you do have the drivers installed then it may be because of what's happened to your phone and then you would need to try and flash from your SD card with the file properly named for your version so that fastboot on your phone will recognize and flash it. I don't know what the proper name is for your version but a lot of the time it's already properly named. How did you lose your recovery in the first place with a locked boatloader and no root?
I must be getting tired. I believe you need an unlocked boatloader to flash a recovery image so you might not be able to flash it. If you can't get into your recovery thru H boot, press power button, volume button to select recovery, enter by pressing power button, then your going to need to flash the stock RUU or zip file with the stock recovery included in the file as you can flash an RUU with the boatloader locked. The issue is will the signed file recognize your firmware as it is now and flash? Wish this was my expertise but I've only flashed RUU files to start my devices clean again not to fix a problem.

I know my friend I read it the first time and I understand all that. I'd try the flash with your recovery zip and see if that works. If not then you'll need to flash stock RUU's until you find one that will work but only ones meant for your version . Did you click that one link I gave that was to a forum here in XDA because that forum looked like it was very similar to your problem ?

Rename it to 0P9CIMG.zip
Copy it to your sdcard
Put the sdcard into your phone
Boot into HBoot, it will recognize an update and confirm
After this, you will be on stock kitkat, but you can make upgrade to lollipop

That's exactly what happened to my device. RUU is still uploading.

yes but im not sure having usb debugging ....only fastboot recognize device adb dont ;] how to install recovery with no debugging but..if RUU dont give me fail pu will be nice
My fastboot didn't work either, but I got the device working again after flashing stock RUU.

Thanks for the assist TobiBot. I don't have this version of phone and didn't catch this forum link when I was looking thru XDA. O.P. says he is flashing this way and not from ADB/fastboot and still keeps getting the" older version" notice. I read the whole forum you linked to and found this a few pages in. His radio I.D. is invalid in Getvar all he posted so maybe your suggestion combined with the above posters quote he can make it work. Hopefully just the fix you linked to will do it but if not this might get him the rest of the way.

The last quote above from the Misctool suggestion. Never tried it or heard of it but all options should be looked at to fix O.P.'s problem. Hopefully the last poster's help fixes it and this idea is moot but at least it's linked here for the next guy.
