Bricked?! Help - One (M9) Q&A, Help & Troubleshooting

Looking for assistance.
Will try and keep this short and simple, but basically my phone is unable to boot and gets stuck in a loop. This happened after having to hard reboot the phone after my daughter enetered kid mode on the latest ICE Rom.
I initially chose to run a custom rom after various attempts to update my phone to nougat via an ota, which was failing all the time despite another recent carrier update to fix the issue.
Unfortuantely I was hasty in doing so and never took note of my cid/mid information and running the commands via adb or fastboot won't returns anything... I'm a bit stuck.
As it stands:
Softare status: Modified
Unlocked
S-ON
I have access to recovery mode in the form of TWRP 3.1.0-0. Any attempts to install a rom fail at various stages (various roms) and any attempts to wipe and reformat also fail with "Failed to mount 'storage'", "Failed to mount 'cache'", data etc. etc.
If I attempt to boot to download mode I get a message stating 'Failed to boot to download mode'.
I looked to try and reformat using adb and pushing ext4fs but it failed. Equally I tried to work out which RUU I need to try and go back to a factory image but I'm unsure which file or img I need.
Could it be a nand issue? If it helps the phone was from EE in the uk and was running on the most recent software version prior to the nougat update.

Recovery induced reboot loop - help!

I need help understanding why I'm stuck in reboot loop with my EVO 4G.
I originally was trying to root the device using unrevoked3 but getting the ERROR "Backup CID is Missing" message and unable to proceed. I followed the instructions at http://androidforums.com/evo-4g-all-things-root/194918-rooting-dummies.html which talk about troubleshooting this. It talks about going into recovery mode to see if I've lost the recovery mode which I did (black screen with phone and read arrows). It says to reboot but I was unable to so I unplugged the battery.
Since then I have been stuck in a mode where the device boots but the sdcard is not recognized and upon completion of boot up it shuts down and vibrates five times. I did a bunch of research last night on understanding what I thought I needed to do alleviate this. My thought was since I was stuck without a recovery image I need to flash new one. At this point I'm fine with not being rooted with all the trouble I've had.
I downloaded the latest stock recovery rom (3.70.651.1) from http://androidforums.com/evo-4g-all-things-root/259446-clockwork-amon-ra-pc36img-files.html and pushed it to my sdcard under the name PC36IMG.zip (with two files .info and recovery.img). I restarted my phone via HBOOT and it sees the zip file, reads it in, goes to checking and then does nothing (back to HBOOT screen). I don't get what I'm doing wrong. I also tried getting the stock image from http://forum.xda-developers.com/showthread.php?t=874091 but I can't seem to open the firmware.zip file to try that recovery image.
Again I just want to get back to stock ROM and recovery files now. Any help would be greatly appreciated!

posible brick, cannot pass google account check after firmware update

Hi.
International version, Europe, S-off, Unlocked.
I think that I managed to write a wrong firmware on a stock, but rooted rom (im not shure if this is the cause, i followed the instructions and the log was ok). The result is that if I flash a rom (with TWRP), no matter which (L or MM), the phone boots, the initial setup starts, language selection ok, wireless connection functional, but when i reach the google account checking, even the email and password are ok, it continuously throw me back to the google email checking...
The situation now is like this:
- can enter TWRP, fully functional
- can enter download mode
- can enter bootloader mode.
The problem is that the phone is recognized by the computer only when in TWRP, no connection on download/bootloader. So I cannot use adb to push commands. When in TWRP i can put anything on internal or external storage and flash...
Please, help me with some advice in order to fix this.

Dont bother. Found the lollipop oficial ruu zip, flashed from the external card in download mode, system fully operational in 5.1 now.
Its an odd one to say the least. I had a similar issue when rooting my phone last night. After installing twrp I couldn't speak the phone for love nor money.
Tried a sideload in twrp and it told me that my adb.exe was too old for my phone so I updated it and then I had adb control at least.
Changed my twrp from newer to older and then everything seemed ok.

HELP! restoring stock rom on Verizon galaxy S5 (G900V)

First of all to admins..., I hope I'm posting this in the right section. If I'm not, please move it or direct me.
I have a Verizon S5 - G900V that is soft bricked and I have been pulling my hair out trying to restore stock rom to it via Odin. It fails every time. I have tried so many versions - some fail with aboot, which I understand means that th version I'm flashing is older and I can't downgrade. Fair enough. The only one that seems to work is G900VVRU2DPD1 (which shows up as the version in recovery mode) but it fails at loading the SYSTEM stage. In Odin on Windows it shows fail (AUTH) an on t device it shows secure check fail: system
Reactivation lock on the device in download mode shows OFF And I can't boot into the OS to verify that.
I have all the necessary device drivers installed in windows.
The device, I believe, is not rooted.
Using kies doesn't work because I asks for for device serial which I d know and does n show under the battery (only shows IMEI an model) and using the Verizon repair Assistant doesn't even see the device
I have wiped cache partition and did factory reset from recovery mode.
I have done exhaustive Google searches an been on many xda dev posts for weeks and not finding anything that works.
I've also tried pushing with adb sideload....didnt work.
The only thing I can think of, and think I might have read somewhere, is that my problems are caused by a locked bootloader. I anyone here able to attest to that?
One other thing...in recovery mode, if I select mount system, it gives errors in red e:Failed to mount /system (invalid argument).
Any experts here able to help me get passed this to successfully flash stock rom (or any rom even modified) back into this device and end my madness?

I have no clue of what just happened

Hello everybody this morning I've encountered something very strange, I woke up and saw a unusual page on my phone that said "Your phone is encrypted" it told me to put in the passcode that is used on the lock screen to DECRYPT my data when I did so the phone without needing any time to think told me that the decrypt of the data was unsuccessful and that the only way is to factory reset
Now the thing is that I have twrp installed so the phone didn't even reboot to recovery it just rebooted and the same page appeared again telling me to input my passcode
What I've attempted to do is to flash the ruu for my device from sd but the phone doesn't see the file
I've tried to flash a different recovery as twrp was unable to mount any partition except the system partition and the SD partition
I will try to flash cm12 from twrp and see how that goes but until then does anyone have any clue of what happened with my device?
Update: twrp cannot boot the system partition either so now I guess I'm officially bricked
Update 2: Looking at bootloader it sees the ruu on the sd but skips it as it says "No image or wrong image!" under each one except 0PKVIMG.zip

I think you can run RUU only from pc, might be wrong tho as I've run only 1 RUU in my life
Have you tried restoring a backup of your current rom? If you don't have one you can also try one from here (read instruction tho, as you need to same cid as your phone)
Can you use the phone at all? Like go past the pin screen? In settings > security you can disable it (i think its there)

it has recently happened to my mom's phone as well. I can't boot into recovery. It just boots straight to the OS and asks for the encryption key. I've tried to reload the stock recovery image, but it failed with wrong signature message. I've also tried unlocking the bootloader with my htcdev bin file, but it doesn't work anymore. I've tried the RUU that I previously used to go back to stock, but it fails with wrong signature as well. It appears the phone is completely bricked. BTW, The phone was relocked, S-On and with stock recovery.
I'm afraid this is a new ransomware, because before that happened my mom said something about message asking her to make a payment to unlock the phone... Is it possible those dbags were able to swap the certificate?

Question Failed to load/authenticate boot image

Good Day All, I've seen a few thread regarding this but nothing stating what causes it to happen. Can anyone provide any insight?
I was about to perform a reflash of my rom but didn't even reach that far.
I restarted phone to recovery
Was going flash a firmware before the rom flash but decided to switch from slot a to b cuz i wasn't sure if the firmware is a global thing or per slot
Changed slots and restarted to recovery
Noticed no prompt for decryption key and my data/media was gone.
Changed slots again without doing anything and restarted (TWRP prompted no OS found)
After the recovery started again no prompt for decryption key again and no data
Figured twrp loaded badly or something. Restarted to recovery one more time
Phone did not boot to recovery. Keep bootlooping to fastboot
Attempted to reflash TWRP when I encounted the error "Failed to load/authenticate boot image"
After a quick google the solution seemed to be the flash over the stock rom via miflash and start over which I unfortunately did
While the solution worked. Does anyone have any clue why this might've happened so I can avoid it in the future? I don't think the Rom matters but I was running Arrow-v12.1-alioth-OFFICIAL-20220819-VANILLA.zip and the recommended firmware V13.0.4.0.SKHMIXM
