connect the phone in "Disc Mode" copy all your data onto your computer, disconnect, change memory card to the new one, and copy and paste to the 16 gig.
Sorry I don't know any way of moving you games or apps when changing an SD card

Same issue, I am changing from 8GB Micro SD to a 16GB micro SD, the catch: I am using A2SD+ and my previous 8GB has ext3 partition, so do I simply backup, change card and copy back the files or do I need to partition the new 16GB card like I did the old one, and then copy my backup?
if you want to save your ext as well, then this is best done via amon-ra recovery.
do a nandroid + ext backup of your rom with your old sd card.
put in a new sd card, formatted with ext, restore nandroid + ext then your rom and ext will be fine.
As for the fat you will have to copy as outlined above (via a pc).

isnt it that the the nandroid backup will be stored in the SD? if so then after I do a nandroid backup, place the new blank SD and do a nandroid restore, then if it looks for any backup, then it wouldnt find anything because the old card isnt there?

The folder "clockworkmod" stores the Nand backups. You need to copy this to the root of your new sd card. This will be detected by Clockworkmod automatically

This is what I did them I got a new card, I had this in another post so just cut and pasted, my solution to moving to a new card. Bering a new member I can't post the link in here, but here is the copy of it here.
My setup, 2GB no class listed MicroSD, HTC Desire, Rooted with MoDaCo r9, with A2SD. All of my apps are on the MicroSD pretty much.
Well the MicroSD came in and I did what I know how to do, I used a cloning program that I use all the time for PCs, figured what the hell it should work.
- I plugged both of the MicoSDs using the SD adapter, then plugged the SD adapter into an SD to USB adapter and plugged them into the PC.
- I used Acronis True Image 2010 Acronis True Image , did an exact clone (called "Disk Clone" in the Acronis software), from my 2GB MicroSD to the 16GB MicroSD. Did NOT adjust any partitions or anything, had 14GB of free unused, unpartitioned space on the new card.
- Then I took the new 16GB MicroSD and plugged it back into my phone, booted, and it worked great, the phone can't even tell that it's a new MicroSD, shows same sizes and everything, as I didn't adjust any partition sizes yet.
- Then I took the MicroSD back out, plugged it back into the PC, then used another product of Acronis, called Disk Director Disk Director and then went in and expanded/resized the FAT partition to the full size, it used the free unused space to do that, and moved the partitions down the line and expands the FAT one in place.
- Plugged the card back into my phone, booted, worked great, got 15 GB plus of space now on the FAT partition. All the apps works, everything works great. It seems a little snappier and faster when loading apps now.
Side notes:
I just happened to have both of these products installed on my PC and use them all the time so that's what I used, but any Disk Cloning program that supports USB will work.
There is plenty of other "cloning" programs out there, like Norton Ghost - not free, some other pais ones. But also bunch free ones as well a list of them here , wiki list of them as well. Some of these programs are LiveCDs, which means you boot your PC of a CD and then you can edit HDs, also the USB Drives, other like Acronis will have a PC program as well a boot disk. With acorns I was able to use it just as a program no booting of a CD.
I personally have used this program called "hdclone" which is free, it has one limitations but it really is not a big deal in this case, it limits the copying to 1 GB per minute, hell for 16GB card thats 16 minutes !!! so not a limitation really. So do a exact clone using HDclone, don't resize the new SD just leave it as is, this worked for me, I think leaves everything exact as possible. Another free cloning program I have used in the past is DriveImage XML , so either of these will work to clone one SD to another. Now to resize, the product I use ALL the time to resize drives is "gparted", this one is a boot CD, so all you gotta do, plug in your SD card using the USB, boot of this small GParted CD, select your drive and then expand the FAT partition. Few extra steps with some free products but it works if you're not able to acquire Acronis.

I just went in to recovery mode with CWM and did a Nandroid back up. Everything seemed to go well. Then i selected Reboot and now the phone is looping with the White HTC sreen and nothing else....... what should I do to fix it?

It's a known issue with Clockworkmod. Go back into Clockworkmod once again and restore the backup you just made. All will be well.

what can i say? very detailed!
the only problem is that, I had to download Acronis product demos before I can do all these. I do imaging before but I dont resize EXT partitions...
ANyway Im still downloading..
Although I think it would help if anybody can post free and lighter (below 50MB, I guess) of softwares that can do USB cloning (disk cloning in general),..
I will post here my results..

So far, this worked for me.
1. Backed-up my old 8GB card on my PC (the FAT partition).
2. Did Nandroid + Titanium Back up. (The Nandroid will backup the EXT partition)
3. Formatted the new 16GB with FAT32 on my PC
4. Resized the 15.++ GB of FAT32 to 14.XX GB so that the 1GB goes to the EXT3- I did this via a bootable GPARTED CD I got here: http://gparted.sourceforge.net/download.php
5. Copied the Clockwork MOD folder from my PC backup (see step 1) to my new 16GB card.
6. Inserted the new 16GB to my phone and booted into ClockworkMOD recovery
7. Did a NANDROID Restore.
8. After restore, I had to shut off my mobile and I removed the card coz I needed to copy back the whole FAT partition backup from step 1 to the new card.
9. Placed the new 16GB back and voila!
(NOTE: steps can be interchanged,for example, I think the copying of the FAT partition done in step 8 can be done after step 3 above.
Anyways, hope it helped.

please bear with me, i am a newbie, have this unrooted desire froyo. i still have no plans of rooting it.
i also wanted to change the 2gig memory card to an 8 gig mem card. can i just follow the instructions above? is rooting really required for this?

Don't worry, you don't need root and it's really simple as copy and paste.

I was using my 4gb sd card for apps to sd for my previously-rooted android phone, and had about a 500mb of it partitioned to ext3. I have since unrooted my android phone and put it back to stock, but in doing so, I lost the recovery console that would allow me to unpartition my sd card. Is there a safe program that will recognize the ext3 partition and remove it?
Gparted. You can use a live cd or a VM.
Hi there. I downloaded a free program call minitool partition wizard. Do a google search for it (I can't post links)
It has worked well for me with my SD card partitioning.
Make sure you back up the contents of your SD card before removing the ext 3 partition as it will delete all the contents on it.

I don't know if Windows recognises it at all but worth a try really. Gparted is a live cd tool that allows you make partitons on a storage device. I think that VM refers to a Virtual Machine, using something like VMWare or VirtualBox.

Well, since you really don't need the ext3 partition anymore, you could format the whole SD card from windows (do a back-up of the content on the hdd before, though). Then in Windows (I'm guessing you're using windows ), with the phone connected in mass storage mode, right click on My Computer / Manage / click on Disk Management and you should see in the list of the drives your SD card (pay attention to the size). Right click on the 512 MB partition - Delete volume... After that you should create a new partiotion, format it as FAT32... Or you could delete all the partitions of the card and create a new volume.

i did that. in disk management i only see a 3.22gb partition. (i have a 4gb card, so i 512mb is still missing). i cannot find the 512mb ext3 partition anywhere. please can someone help me to find a software or any other method which will be able to detect the ext3 partition as well ?
The partition may be corrupted. You can still try to use Gparted live cd - google for gparted-live-0.7.1-5.zip . It can also be installed on USB flash, check their site for instructions. Restart your PC and boot from the CD or USB flash then check your sd card. If it still doesn't recognize the 512 partition, you might be out of luck.

how could the partition be corrupted because i was using the ext3 partition to install apps when i had root. it was only recently that i have to sell out my htc desire and i unrooted my htc desire. unroot procedure was done successfully but the card is only showing 3.22gb, i-e, 512mb is missing. this may become an issue if the buyer does not want to pay for a 4gb card with only 3.22gb of storage. it should be around 3.5-3.6gb
A partition may become corrupted if you remove the battery from the phone while the system is still running (i.e. writing data on the partition), it is not unheard of. Do try the gparted solution mentioned before, though.

yes buddy i have tried the gparted solution you suggested before. even that is showing only 3.22gb of my sdcard and no mention of the 512mb ext3 partition. i dont know whether i am doing something wrong or what.... ???
Anyway, is there anyway i could recover that corrupt partition? Like can i ROOT my htc desire again and then try to format the ext3 partition from the recovery? i read somewhere there is a way to do that in recovery by giving some commands. and once i have recovered the ext3 partition space then i can UNROOT my htc desire again. DO YOU THINK IF I DO THIS ITS GONNA BE OF ANY USE? Please help me.
This might work since it worked for me. 4GB mysteriously disappeared from my 16GB card and neither Windows or gparted picked it up. The only way I was able to get it back was putting it in my phone and the Android picked it up. I did have to format my whole SD card but nontheless, I got all my space back. Hope this helps and if you have tried this then I can only guess that your SD card is buggered.

um actually, my sdcard IS in the phone this whole time as i dont have any card reader. i am connecting my phone with my pc in hard drive mode.
from within the phone, i have tried to format the sd card many times but the space does not go beyond 3.22gb..
Something seems to have messed the partition up. Have a look around on the web

because there is a way to delete the ext3 partition from the recovery.
finally recovered my ext3 partition.
this is what i did.
1) rooted my phone.
2) enter into clockworkmod recovery
3) mount sd-ext (i guess it wasnt mounted that's why no app was recognizing it)
4) now windows was able to recognize the ext3 partition.
5) problem solved.

i had the same issue as the OP
Use a card reader and MiniTool Partition Wizard.
Delete all partitions on the SD card and create a new one that fills the entire SD
Due to size limitations (GPads only coming in 16gb varieties) I am beginning to run low on space on my Gpad (v500 running cm11). However I have a 64gb microSD card and I would like to use this as much as I can to conserve space on the internal memory.
There are a few ways to solve this but I not fully know how to do this.
Firstly, is it possible install apps to microSD card? If so, how? I know some devices support this and some do not. My phone for example does not but I have heard that some people have had success with this.
Next, there are some rather large folders on my internal storage that I would like move to my sdcard however the apps that use those folder can only read them where they are currently located (cmupdates, ebooks, etc).
I read about symlinks and managed to make a few but the only place I was able to make a symlink was to /data/media/0. But the changes to that do not show up the apps which I believe read either /sdcard or /storage/emulated/0. And for my SDcard I can get to it either in /mnt/media/sdcard1 or through /storage/sdcard1.
I tried and succeeded in making a link between /mnt/media/sdcard1/eBooks and /data/media/0/eBooks however the link does not show up in /sdcard or /storage/emulated/0 and it does not show up in the apps, leading me to believe that is where the apps read the sdcard from.
If anyone can answer the above two questions or give any other suggestions for maximizing available space on my internal card please let me know.

I think most of your issues could be resolved with Link2SD (under 10 posts, can't post a link to Google Play).
You can also use an app called Foldermount. It's only $.99 ( if I recall correctly) for the unlocked version. This allows you to move any folder to SD card. I like it as it guides you thru the way and will move the data for you as well. Latest version has been updated to allow apps from play store to install properly. Well worth the $1 price.

Thanks guys! Looks like using these two combined I can get exactly what I wanted. I'll use link2SD to put apps on the SD card and Foldermount to move the other folders I want moved.

Can you let me know if this works for you? I can't get mine to read or write my SD card at all! I can't even move a picture from my Box cloud account to the SD card. Are you experiencing this? Or did I get a bad unit?

These solutions did work for me, I am still tweaking to get it the way I like it especially since every time I update my Rom Link2SD has to generate new scripts. I can ready and write to the SD card just fine and I could before I started using this as well. I am not sure why you aren't able to write to your sd card. Could it be a bad SD card? Try plugging it into your computer.
I've tried 2 different SD cards both of which I can move files to and from on my computer. So I know it's not the card. I'm thinking maybe I should exchange it out for another day one...

Probably... That sounds like a device issue not card.
How are the cards formatted?
The ROM you are using may not support the format.

I'm not sure I just put the card in my computer and clicked format SD card. I'm not rooted or running any special ROM, it's completely stock out've the box.
Well, if you did it from your PC, it may be formatted NTFS, and you need them formatted FAT 32. Get it here; http://www.partitionwizard.com/free-partition-manager.html Not only allows formatting almost any format xFAT, Fat32, NTFS, etc.. and you can partition them also which is not supported by Windows, but IS supported by most Android ROMs. I doubt there is anything wrong with your device.

Thanks for the tip, this makes me feel a little better about my purchase, I'll give it a try. I'm also thinking of buying a 64gb San Disk card and hopefully it will just work with a brand new one.

I have been trying to fix the issue with Rom updates and link2sd. You have to provide the Rom (in my case cyanogenmod) a backup script so it can back up the files. I am planning on figuring it out at some point but if you get it before I do would you mind posting your script? Thanks!

So a follow up with this, I was able to get a script working. I don't know if this feature is unique to cyanogenmod or not but any script in /system/addon.d will execute itself during a rom install and be restored automatically if it was wiped. So using 50-cm.sh as an example I made a script called 11-link2sd.sh to perform the backup of the mount script for link2sd which is located in /system/etc/init.d/11link2sd.
I then had insert this script and my mount script into a cyanogenmod update and flash it in. But after that I tried flashing an update of cm11 to test it and it worked! Attached is my script which should also work for anyone who is able to flash it into their /system/addon.d folder.
So, yea, i bricked my S3... it happened when I was installing a custom ROM HOLD UPP!! It was a ROM that was for my device, and it wasn't caused by flashing the ROM. I formatted /system by accident (yea, I was using TWRP), but the strange thing was, it allowed me to boot once. before I actually went into the 'bricked' state. When I realized I formatted the /sdcard directory, where the custom ROM was stored (yea, call me an idiot), I rebooted so I can reboot to use the browser to download the ROM, but it stayed at this screen, except it didn't say "I9300"

Then I realized that TWRP had an MTP feature so I can transfer the ROM from my computer. So I transferred the ROM, flashed it, and yes, I did a full NANDroid backup, and stored it on my PC. After I clicked reboot, it got into the bricked state. the only signs of life, is when the LED turned red when I plug it in, but that only happens when I take the battery out first, and the phone gets warm when I plug it in.
So I need to know how to unbrick, without a 16 GB SD card. i only have a 2 GB SD. Is there a method to debrick using SD cards less than 16 GB? Is there a way to transfer the boot files to the phone? The phone, when I plug it in to the computer, the computer actually recognizes it, but as qhsusb_dload. what does that mean?

It sounds like you found the debrick thread so I won't go into that. To answer your questions though, aside from that debrick method, the only other option is a jtag repair. There may be a shop near you that can do this, but if not you'll have to send it somewhere.
As for the size of the SD card for the debrick.img, I think I remember hearing once or twice that an 8gb card worked, but I still question if that was true. The idea behind it is the SD card is supposed to be the same size as the devices internal storage, so nothing less than 16gb will work. Can't hurt it to try of course, but don't get your hopes up is all.

It's a definite indicator that a device is hard bricked.
For future reference, I recommend NEVER wiping /system. When you flash a rom that's the first thing that happens anyway, so doing so manually has zero benefit, but all the risk of ending up with a device that has no O/S to boot into.

The procedure recommends to use a class 10 16GB micro-SD card only, but I managed with a class 4. I really think you ought to try with your 2GB card, as the debrick image is only 80MB. The worst that could happen is that it simply won't boot off the SD card.
In all my browsing on XDA I couldn't make sure to find a certified I747M debrick image, so I made one from my wife's rooted S3 (I747MVLUFNE6) following instructions from this post, and used it to boot mine (I747MVLUFNK2). I used Win32DiskImager (link to it is in the post) to get the image on my SD card. After I got it to boot, I downloaded the stock firmware for my device and used Odin to flash it. It worked just as they said.
I recommend you use Win32DiskImager to backup your SD before writing the debrick image : this will save you 1) copying the files manually and 2) looking for some tool (e.g. SD Formatter) to restore it to its former size (after writing the image, Windows will think the capacity of the SD card is 80MB!).

To me its look like, you get stuck in boot...

I flashed the image through my firends phone instead of a sd card reader
so my friend has a kyrocera rise, so i inserted my sd inside, and it showed up the sd card in my pc, and flashed the img, but it didnt work. yea i think the theory about the sd card having to be the same size as the internal storage is true. im going to try writing the img using the reader instead. yea i already backed up the few files i had so that wasnt an issue!

Not to be rude or anything, but i know what to do about soft bricks. I'm not in that situation. and i have a i747M. that means its Canadian. i also stated earlier it is from rogers. what does having to be a at&t customer have to do with anything, if this is a 2012 phone? The warranty is gone already.

Yea i wiped /system by accident. but, it allowed me to reboot AFTER i wiped /system, which was weird, and i thought you were supposed to wipe /system. I wonder why the option to wipe /system is there anyway? Also, i flashed the rom already, so wouldnt the /system be written to the parition already? i thought there was a /boot partition..
I can get my hands on a 16 gb sd cad by sunday, but i was just looking for a unbrick method for a 2 gb sd card. Even if it means i have to keep the (2gb)sd in my pone in order for it to boot, i was looking for a temporary method to unbrick my phone.

It's a very common mistake. I see it happen way too often. I don't know where or why it got started, but some folks always recommend wiping /system. (Maybe it was required on a specific older device and they carried over to others).
tbh, I can't think of any situation where it's beneficial, so I don't know why is still included in recovery options.
Not sure why it would've booted that one time. I've seen that happen before, but only when flashing the firmware, not just a rom.
If the flash was successful after transferring the new rom via twrp, then you are correct that that the /system partition wouldn't be empty anymore. Maybe the rom was corrupted during the download or transfer?
The boot partition is actually where the kernel is located.

Like I mentioned before, don't get your hopes up using the 2gb card. But by all means give it a shot! Can't hurt anything to try.
Someone mentioned the class of the SD is important. It is recommended to use a class 10, brand name card, but it's not required. I've seen class 4 cards work where class 10 cards failed. I believe it has to do with the transfer speed if the individual SD card. Some class 4/6 cards can actually read/write much faster. Just like some class 10 cards just don't perform as advertised.
Still, the most successful ones have been class 10's, which is why I think many consider it a requirement. Many lower class ones do often fail, but not all. Just something to keep in mind...

Not that I've ever heard. But I doubt many have even tried. I've only seen a couple of reports that it worked with an 8gb card, but as I think I mentioned before, I'm not quite convinced that it was even true.

Flashing stock firmware to my phone after booting off the SD card into download mode restored the phone's bootloader, but I knew that it was the bootloader I'd messed up.
The posts I've read about debricking this way seemed to mention that the phone would use the bootloader from the SD card, but would finish the booting process from the phone's storage.
If it's really /system you wiped, I wonder what your phone could finish booting from.

i think it was because it was given a direct command to be booted into recovery, instead of being booted into system. :silly:
Im worried when i use the 16gb sd card, THAT wont debrick my phone! All i did was formal /system by accident, and it booted again AFTER i deleted system.
Also. I'm a noob at Ubuntu and all the "of=/dev/block/mmblk1". I thought it was as simple as
1. insert 16gb SD card and open Win32DiskImager
2. select the debrick.img and flash it
3. insert the SD in the phone and attempt to start it in download mode
BTW i heard that the CPU varient (exynos, quallcom) matters when u debrick the device. i have a SGH-I747M rogers so what CPU variant do i have?

You have a dual core Qualcomm processor. As long as you are using the correct debrick image, or correct bootloader, modem, or ROM when flashing the processor worries take care of themselves.
You could use either method to flash the img to the sdcard. Win32diskImager could be used on Windows PCs. dd is a command line tool used in Linux to do the same. Brasero would be a GUI tool used in Linux if using the command line concerns you. Using the GUI tools you need to be sure you are 'burning' the image to the media instead of copying the image file. The same would apply to making a bootable usb thumb drive to create a live Linux drive.

Unfortunately, it sometimes does not work. More unfortunate, there is no way to know if it will without trying it.
Earlier though, you described a classic hard brick. Red light, QHSUSB_DLOAD.... this method has fixed bricks just like yours plenty of times. More have succeeded than failed. But as I said, it doesn't always work. I think it's definitely worth trying though.
I am convinced that 90% of the failures were due to two things: the sdcard and/or user error. (Mostly the SD though.)
When you do try this method, keep in mind that many people don't get it to work the first few tries. So if it doesn't work, don't give up right away. Try at least 3 or 4 times. This actually happened to me twice! I did everything exactly the same way but it failed until my 3rd or 4th try! No idea why, but I have experienced this firsthand.

The SD card won't debrick your phone on its own : it will only allow you to boot into download mode. Successfully flashing stock firmware (which contains a bootloader) from download mode will.

BTW, where did you find a I747M debrick image to flash on your 2GB SD? The only ones I could find were for I747 (US)...

But after installing the same my MicroSD card says its corrupted and asks to format it. I have formatted it several times and also by PC with a card reader. But still the issue persists. I don't get the option to set up my card as either INTERNAL or PORTABLE storage as it should be in Marshmallow ROMS. It directly starts to format as Portable storage and starts at 20% and shows successfully formatted. But still it shows that the card is corrupted. My card is working fine. I checked it. Please help.
I have the same problem and not yet identified the reason for this. Have you tried other ROMs? For me it is also happening on CM 13 and CandySix.

I have the same issue with a Samsung 64GB microSD in a LG G3 d851 (T-Mobile USA). The card works fine in Windows and Android Lollipop. However I am testing several Marshmallow ROMs and so far I got this corrupted message in LightDark, Turbo and DU ROMs. However it works fine in Tesla-M. All of them are AOSP Android M ROMs. It seems that there is a problem somewhere but at least Tesla-M tells us that it can be fixed.
Fat32 lads, instead of exFAT format to fat32. The root in MM causes exFAT cards not to be read and stupidly the phone formats it to exFAT. Also the SIM contacts won't appear, instead you have to download an app to copy them from sim to phone.
I'm experiencing something similar.
I am on fulmics 4.2 on a d855. I had no problems for some weeks, than the phone started mounting and unmounting my sdcard, I tried all the suggestions I read online. Like slow formatting (tried all the types of partition), also I tried with the phone format. My best result was the read of the sdcard for just some hours, but then it started auto-expelling it.
I obviously tried with more sdcards and it's not a problem with them. Any suggestions? It's a software problem or an internal hardware problem?
I tried change the kernel a xceed and the sd card is recognized but not funcionally complete,(the radio fm do not work with xceed)

Hi all,
I found out that the same SD card I have since I bought the S5, May 2014, just got corrupted.
It won’t format in the phone, I use the AParted, ES File Explorer, FX Explorer and it never work.
I can still use it in a camera, PC, another Phone and it worked fine!
I put the card back in my PC, reformat in Fat32, ext4, NTFS and each time never worked in the S5.
I did the same thing with a bran new one too, same results.
It starts after updating SuperSU 2.61 to 2.71, in Marshmallow.
Few days’ work fine and then, corrupted.
I tried many options people have here and still the same result, corrupted.
I fix it (I hope) with doing a full factory reset in Marshmallow first.
Got to TWRP and did a full Wipe, Format Data and Adv. Wipe; select everything.
Yes, that clean a lot.
Use Odin and re-flash Marshmallow.
Everything is fine now, but it is only a few days that I did this, so it might still happen.
Could it be the version of SUperSU I took that has something? Or It is actually Marshmallow 6.0.1 with SuperSU ?
We might never know as the version change often.
Hope that will help someone.

So it seams to be a combination of OS 6.0.1 (Canadian Rogers), how it is rooted (Twrp) and the SuperSU free v2.76.
I’ll wipe the phone again and re-install the original Os from the carrier, no Root & no SuperSU.
I’ll wait for a week and see.

Since this is my first post I can't paste the URL to the site as the forum won't let me, so you'll have to google it, sorry about that.
Google - 32fat guiformat and look for the site 'GUI Format - Ridgecrop Consultants Ltd'
Download the program by clicking on the picture.
Run the program and select the drive letter of your memory card. Set the Allocation unit size to 32768 and untick Quick Format the press Start.
The format process takes maybe 45mins. But afterwards you'll have a full 64GB card formatted as FAT32 and will work in your phone again. Hope this helps you...

If you can verify that more than one card works in a computer but not on the phone, and that formatting the card in the phone goes to 20 % very quickly, while afterwards going to 100 % but still tells you it´s corrupted, then indeed it´s simply the installation of Marshmellow that suddenly no longer wants to work with the file system of the card.
For me, the 32fat guiformat program told me that card was locked by another application, so I did it manually:
In windows elevated command prompt - after backing up eventual data on the card:
diskpart
list disk
(now find out which one is your sdcard, let´s call it x)
select disk x
clean
create partition primary
select partition 1
format fs=fat32
When the format is finished, if the cards size is beyond 32 GB, you´ll get a message about that the disk is too big.
At this point I managed though to make a quick format in the before mentioned FAT32 GUI Format program, and bingo - the card is recognized in the phone again.
No need for the diskpart stuff of course, if you already had success with the GUI thingy, just telling in case anyone experiences the same problems as me.
It may be that there´s a way to tell diskpart to use a different cluster size to avoid the error at the end, and also there may be a trick to make the card available to fat32gui. without first going through the diskpart step In my case though, I had the error on two different computers, on which none of them were accessing the card from any applications, disk management, win explorer etc.
Finally I´d like to add that this is not a general error for all Marshmallow phones: My huawei p8 worked fine with the card, also on Marshmallow, before I had to reformat it to fat32.
It´s a pretty sucky bug: Never mind the hassle of formatting the card, as long it works. What bothers me is the 4 GB limit of files in fat32. It´s easy to have a movie nowadays which is larger than that.
PS: Some people recommend using an app called aparted to format the card on the phone, which now has another name. I tried that, but it kept on closing itself, so perhaps it´s not compatible with Marshmallow or the same plus LG G3.

I know this is an old thread but I've recently had this same situation but I found it was a result of installing busybox with Busybox Pro from Stericson. I used the "Smart Install" feature and selected to "Replace all symlinks" and "Clean Mode".
Basically, I believe by doing clean mode and replacing all symlinks, I had replaced needed System applets which somehow had hindered the OS from being able to correctly mount any External SD Card.
After using TWRP to factory reset (Wipe System, Data, Dalvik, Cache, Internal Storage) I Odin Flashed the latest stock firmware and then proceeded to root, etc. BTW, My phone is a T-Mobile Samsung Galaxy Note 4 SM-N910T on Net10 prepaid network.
Worked for me!!

Did you ever find out a resolution to this? I'm having the same problem -- card works in computer and other devices, but not tablet which was rooted (twrp) with supersu installed. In fact, no sdcard will work now in that tablet, formatted using gui formatter mentioned earlier or not.

I know this is an old thread but I've recently had this same situation but I found it was a result of installing busybox with Busybox Pro from Stericson. I used the "Smart Install" feature and selected to "Replace all symlinks" and "Clean Mode".
Basically, I believe by doing clean mode and replacing all symlinks, I had replaced needed System applets which somehow had hindered the OS from being able to correctly mount any External SD Card.
After using TWRP to factory reset (Wipe System, Data, Dalvik, Cache, Internal Storage) I Odin Flashed the latest stock firmware and then proceeded to root, etc. BTW, My phone is a T-Mobile Samsung Galaxy Note 4 SM-N910T on Net10 prepaid network.
